New Patient Registration

Patients wishing to register with the practice, please contact the surgery to check if you are in our area first.  Eligibility is based purely on Geographical area not necessarily your postcode. The practice is currently only accepting patients that have moved into our practice area.

If you are eligible to register at the practice, you will need to complete a registration form. We will also ask you to book an appointment to be seen by one of our practice nurses for a new patient health check.

Please Note: The registration process will not be complete until you have attended your appointment with the practice nurse. Until that time you will continue to be registered with your current GP.

Completing your Registration request online

If you wish to request to register click on the link below to open the form, fill it out and bring it in with you on your first visit to the practice along with 2 forms of identification – one a photo, the other must have the address of where you are registering. You will then need to contact the practice the next working day to find out if your registration request has been accepted.

If your request has been accepted you will need to make an appointment to attend for a new patient health check with the practice nurse. We would ask that you bring any medication you are currently taking with you to this appointment.

Temporary Registration

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from a local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered. To register, you will need 2 forms of identification – a photo ID and something with the address of where you are staying.
