Ordering your prescription by email at - The practice recognises that many patients that do not have an online access account are concerned about ordering their prescription. We have therefore set up an dedicated email account to receive prescription requests. Requests can be accepted directly from the patient or on behalf of the patient from a relative, friend or carer if appropriate. All requests should include full patient details; Name, Address, Date of birth and all items required (please note we will be unable to respond to requests which detail 'everything'. Items must be named. We will process requests via email within the same 48 hour time frame as long as staffing numbers at the practice allow. We are unable to process requests early and ask that patients do not request for the purposes of stock piling. Please include the name of the pharmacy you would like your prescription sending to and we will arrange for it to be forwarded directly to them.


If you are asked to have an ECG you will be given a request form to attend the Cardiology Department at Cannock Chase Hospital.

The hospital offer a walk in service Monday to Friday. The times the department is open will be on the request form.

Once your ECG has been completed you will be handed the result to return to the practice. We would ask that you bring this result straight back to the Practice. If the referring Doctor is still on the premises we may ask that you take a seat whilst we show them the result. Alternatively we will make sure we have an up to date contact number for you and the Doctor or receptionist will contact you once the result has been seen.
